• File a complaint. You can complain to us or to the federal government if you believe your privacy rights were violated. You will not be penalized for filing a complaint.

Our responsibilities

We are required by law to keep your PHI private, give you this notice, and follow the terms of the notice currently in effect. We may change this notice and apply the change to information we already hold. If we make a material change, we will post the updated notice in the pharmacy and on this website with a new effective date.
